Sample Submission Forms
Please use our CoreOmics online submission system to submit samples. An electronic copy will be sent to us once a submission is created. Please print a copy of the submission (landscape orientation; all on one page) and include it with your samples.
Make sure your sample name matches what is written on the top of the tube. Sample names must be between 2 and 12 characters, uppercase letters ONLY, numbers, and underscores.
